Deadline to submit fall sports paperwork is:  Monday June 9th 2025

Paperwork can be placed in the mailbox of the sport you are interested located in the main office!

FALL SPORTS: Football, Boys Soccer, Girls Soccer, Cross Country, Girls Volleyball, Girls Tennis, & Cheer

ATHLETES: Don't forget to add yourself to the Google Classroom of the sport you are interested in for updates, try out info etc.

1.  Pre-Participation Physical Evaluation

(Filled out by doctor, stays with doctor)

2. PPE Medical Eligibility Form

(Filled out by doctor, gets brought to school nurse completed)

3. Health History Form 

(Filled out by parent, stays with doctor)

4. Heath History Update

(Filled out by parent, goes to School Nurse )

5. Athletic Acknowledgement And Consent Form

(Filled out by parent, gets brought to head coach completed)

If an athlete does not have an up-to-date sports physical in the last 365 days, they will require new physical form.
